We see Daryl and Beth are together on their own; Tyreese, Lizzie, Mika and baby Judith are together on their own; Maggie, Sasha and Bob are together and Maggie wants to look for Glenn; Glenn wakes up at the prison and finds Tara. Tara says that her sister Lilly was swarmed by walkers. She also tells Glenn about Hershel getting killed. Glenn wants Tara to join him in searching for Maggie.

Lizzie cuts up some rabbits with her knife and it looks like she later smothers baby Judith a little but doesn’t kill her. So we can assume that Lizzie was the one who was feeding rats to the walkers at the prison. We see that Tyreese has an injury to his wrist/arm area, I believe his left one. While Tyreese goes to help two strangers, Lizzie and Mika had baby Judith and had a little walker trouble but Carol saved them. Carol then brought the girls to Tyreese and the man Tyreese tried to help told them to stay on the tracks as it will lead them to a safe place. They find a sign that says “Sanctuary for all.”

Glenn and Tara fight off some walkers where Glenn passes out. An army truck pulls up as Tara is killing a walker. Abraham, Rosita and Eugene are then seen standing in front of the truck.
